Tutoring Registration

You Consent As Follows:

● I am the legal parent or guardian (releaser) of the child participant named above. I hereby give permission for the participant to be photographed (with or without other classmates in a particular picture).

● I understand, agree and give permission for City Kid Explorer to display the photographs on the www.citykidexplorer.com website.

● I understand, agree and give permission for City Kid Explorer to use the photographs for promotional purposes.

● I understand, agree and give permission to City Kid Explorer to share photographs of the child participant with other participants and their families through a shared folder on Google Drive.
